Care managers are nurses, social workers, and other licensed clinical staff who can help you set and meet health goals. They also partner with your entire medical care team so everyone is on the same page.
I am a Native North Carolinian—go heels! Much of my work has been with individuals and families in direct mental health practice. I have worked with youth in residential care and pregnant and parenting teens, and most recently before coming to Duke, I managed a Behavioral Health Clinic specifically for Medicaid and Uninsured/Underinsured individuals. I received my Bachelors in Social Work from North Carolina State University and my Masters in Social Work from the UNC Chapel Hill. Working with DukeWell will allow me to continue my focus and practice to ensure individuals and families are able to get quality care and treatment without delay.
I have 25 years as a Registered Nurse with a varied background. During the last three years, I have worked in population health and transitional care assisting hospitals, ACOs, and CINs with implementing strategies that positively impact quality of care, including readmissions and monitoring metrics. With the amount of data available, it allows us to be able to identify opportunities earlier that will positively impact care. Population health is integral in the evolution of health care as it allows us to focus on quality while engaging patients and/or their families, leading to better communication, collaboration, and coordination of care.
I was born in Jamaica and relocated to New Jersey. I received my Bachelors Degree from North Carolina Central University and Masters Degree from Howard University. I am currently a Licensed Professional Counselor, and my career experience includes behavioral health services working with children, adolescents, and adults. I enjoy traveling, reading, listening to music, and spending time with my children and family. I have a passion for helping others to be their own best advocates.
I have 14 years of nursing experience with a background in General Surgery, PACU, Pre-Op, and Telephonic Behavioral Health (focusing on depression and anxiety). Nursing is my passion and I truly enjoy making a difference in the lives of those in which I provide care.

"I grew up a Duke fan (I have my Coach K autographed basketball displayed in my home!), but received college degrees from local rivals, NCSU and UNC-CH. I live with my husband and 2 daughters. Boating Is a passion of ours; my favorite trip is to trailer our boat to Florida and drive it to a small island in the Bahamas, Green Turtle Cay. I also love to ski, ride horses and spend time with my dogs. I have a long background in pediatrics and home IV infusion therapy, both pediatric and adult."
"I'm originally from the rolling hills of East Tennessee, but have called Durham home for over a decade. I earned my MSW from UNC-CH in 2006 and my clinical licensure in 2008. After many years in community mental health, I transitioned to medical social work in 2017. In my downtime, I love hiking and camping with my family, trying to keep plants alive and planning my next travel adventure."
I always knew I wanted to help others and ultimately graduated with my Master’s from Rutgers University in NJ. I have worked in pediatric hospitals and schools for most of my career. When I moved to NC, I worked as a case manager in an adult ICU and am very excited about returning to work with the pediatric population. As a care manager, I look forward to helping families navigate the health care system and improving their overall health.

I attended Johns Hopkins University School of Nursing for my BSN and then moved to North Carolina. It was easy to immediately become a Durham and Duke fan! I have been an RN for 22 years, earning my BSN from North Carolina A&T State University: "Go Aggies!" I've worked in a variety of settings and worn lots of hats from bedside nursing to management. I enjoy making a positive impact in the lives of my patients. I work hard to ensure I'm living my best life, and I strive to help my patients do the same.
I am a registered nurse with a background that includes general surgery, postpartum, home healthcare, and most recently pre-op surgical services. I recently earned my BSN. I enjoy hands-on patient care, but educating patients has grown to be an area I enjoy.

I am a North Carolina native and a self-professed “Southern Belle.” I earned my BSN from Winston-Salem State University, and I am currently pursuing my MSN – Nurse Educator from UNC Wilmington. My career experience includes a variety of services, including cardiothoracic surgery, procedural float pool, ambulatory care services, and clinical education. I enjoy traveling, dancing, and hanging out with my two amazing children. I truly have a passion for being a nurse and look forward to sharing and cultivating that in others.

Originally from South Florida, I received both my bachelor’s and master’s degrees in social work from The Florida State University. I moved to NC in 2004 with my husband and son, and I immediately fell in love with the fall and winter seasons (we don’t have them in Florida!). I have worked in the healthcare field since 2001, and I joined the Duke Health system in 2012. My passion has always been healthcare, specifically working closely with patients and families to assist in meeting their needs and reaching their goals. In my free time, I enjoy reading, cooking, running, and traveling with my family.

I was born and raised in Durham, NC. I am a graduate of Winston Salem State University School of Nursing. My nursing career includes experience in ambulatory care as well as geriatrics. I absolutely love being a nurse and advocating for my patients. My goal is for each patient that I encounter to have the best experience possible.
"I grew up in lovely Madison, Wisconsin and then lived in Washington, DC prior to relocating to the Triangle Area. I love exploring the cities, mountains, and beaches of the state. I have family spread across multiple states and even countries, so I do a lot of traveling in order to see them. In my free time I keep it pretty low key- spending time with my husband, son, and friends, reading books, and binge watching the occasional tv show. I love to check out a new restaurant or see live music. I spent the first part of my nursing career working in the hospitals, then got a Master’s in Public Health, and returned to nursing in the community setting which is a natural fit for my interests and blends my nursing and public health backgrounds perfectly."
I grew up in New Jersey but lived in New York City for 10 years and Los Angeles for 14 years before moving to the Raleigh area in 2018. I received my Bachelors in Social Work from West Virginia University and my Masters in Social Work from New York University. I have over 20 years of experience working as a clinical social worker, with the majority of those years working in behavioral health. Prior to coming to DukeWELL, I worked in the Duke Emergency Department. I enjoy working with patients to identify barriers to care and help problem solve so they can be more successful in achieving the goals they have set for themselves. When I am not at work, I enjoy time with my family, photography, baking, and getting outside (hiking and riding my bike)!
"I earned my MSN from Winston-Salem State Univeristy as an advanced nurse educator. Most of my nursing career has been in the perioperative setting as a certified operating room nurse and clinical educator. My experience as a kidney transplant coordinator inspired me to pursue a more patient-centric nursing roleMy most unusual nursing job was teaching health occupations to high school students—what a privilege! My leisure activities center around creating art—photography, painting, pottery, and jewelry making."
I have a diverse nursing background that includes pediatrics (hospital and home health), concurrent review, and over 12 years of Care Management experience. What I love most about my work is finding the right resources for patients who have the greatest needs.
Born in Brooklyn, NY, I am a city girl at heart. I attended New York University for my undergraduate degree, and I lived in NYC until 2005, when my husband convinced me to move south. We originally relocated to the Triangle in 2006 and have formed deep and loving roots in Durham. I began working in community behavioral health in 2007, obtained my Master’s in Social Work in 2013, and received my clinical license in 2015. I have had a variety of roles, both clinical and administrative leadership, and through it all, my one constant has been my love for helping others to meet their goals and achieve holistic health. I am passionate about the work we are doing here at PHMO to help our patients achieve their health goals and overcome the barriers they face. I live in downtown Durham with my husband, daughter, and Chihuahua.
"I was born in Colombia and moved to Canada in 1998 and to the USA in 2007. I am social worker by training, as well as a professional dancer, choreographer and painter. I have two boys and truly enjoy nature, especially the ocean. My family strongly believes in community engagement and we are constantly involved in community activities and projects. I am inspired by people’s tenacity and resilience, they remind me of the blessings we receive every day."
"I was born in Pomona, California, and raised in Durham, North Carolina. My first degree was as a paramedic. Although I enjoyed it, I didn’t think it was exciting enough! My ultimate goal was to be a trauma surgeon, but I’m a nurse and I love it! I enjoy going to new vegan restaurants and spending time with my two dogs, husband and new granddaughter."

"I have worked in direct patient care and specifically with low-income individuals, pregnant women, families and immigrants. Prior to my position at Duke, I worked in HIV/AIDS research at University of San Francisco California and at community clinics overseeing a CDC funded HIV testing project in clubs in Oakland, CA. In a past life I worked in global health across Africa and Latin America. I hold a Masters of Public Health from the UNC Gillings School of Global Public Health. Through these diverse experiences, I have developed a passion for identifying creative and engaging tools to facilitate and promote patient centered engagement in the public health sphere. Outside of work I enjoy gardening, birth doula work, and planning detailed travel itineraries then going on those vacations."

I am a Greensboro native and a graduate of UNC Greensboro, where I received my BS in Nutrition and Health Coach certification. I am pursuing a Master’s of Public Health Nutrition at Appalachian State University. Prior to joining DukeWELL, I spent six years with NC State University, providing nutrition education programs and leading health initiatives to youth and adults across the Triad. I enjoy building relationships with patients and helping them break down complex needs into small, tangible steps towards their healthcare goals! In my free time, I enjoy cooking, hiking, playing competitive kickball, and paddle boarding on the lake.
I was raised in Rocky Mount, NC and attended UNC Greensboro, where I received my bachelors degree in social work. I obtained my Masters of Science in Social Administration (accredited MSW) from Case Western Reserve University in Cleveland, OH. I have worked in a healthcare setting for over 10 years and started with Duke Health System in January of 2014. I enjoy building relationships with patients and hope to continue making a positive impact with their healthcare needs.
I’ve worked in the human services field for over 10 years as an advocate in patient care. I’m truly enriched when I see patients making steps towards positive change in their lives. I’m grateful to have an opportunity to play a part in helping them meet their health care needs.
“I graduated from James Madison University (GO DUKES!) in Harrisonburg, Virginia with a degree in Health Services Administration. My husband and I lived in Washington, D.C. before moving to the Triangle in 2014. We have two children that keep us busy with dance, rowing, sailing, music, and band. Care coordinators connect you to community resources and other DukeWELL team members who can help address transit, housing, and medical care needs. They also contact you when it's time to get care from your medical team that will help you stay healthy.

"I have been in healthcare for over 20 years performing tasks in medical records, inpatient care, billing, front desk registration, laboratory order entry, ancillary scheduling, referrals processing, call center, verification of insurance benefits, alternative medicine reception and more. I re-located to Durham during my college years and have made the Bull City home for me and my family. I enjoy almost all things related to theme and fun parks, dining out to explore many cuisines and traveling to see different climates and cultures."

My healthcare, corporate, and entrepreneurial experience, along with EPIC Ambulatory and Inpatient training and activation support, gives me a unique and practical understanding of healthcare technology and business operations. My background is community outreach/senior resources and providing care for the senior population. I like the type of work I do because I have a passion for assisting people and finding out ways a person can live a better quality of life with limited financial resources or limited family support.
I received my Bachelor of Arts at North Carolina Central University and my Master of Science in Health Services Administration in 2011 and have been in different sectors of health care for over eight years. I have a strong eye to consultative selling style, and I’m someone who is concerned with the patient's needs. I consistently ask questions to uncover the "what" the patient truly wants, and I'm able to meet those requirements (which ultimately produces results). I like to meet challenges head-on rather than sweep them under the rug and hope they go away. I am dedicated to building something larger and someone who puts aside personal growth for the well-being of an organization. Three words to describe me: mature, candid, and honest. Every day I work to improve myself and my skills, which is part of maturing and becoming better at what I do.
I was born and raised in the Great Lake state and am very new to the Triangle area! I received my Bachelors of Science in Interdisciplinary Health Service at Western Michigan University, located in Kalamazoo, Michigan. I have worked in various health care settings during and after college. Specific areas include behavioral health, veteran care, and geriatric care. Ever since I was kid, I knew that I had wanted to help others in any way I could, so I am so excited to be working in a department that will allow me to continue following my passion.

I have been with Duke since 2015. I started with Duke Access Services as an Appointment Coordinator and then a Patient Service Representative. I then transferred to the Duke Eye Center as a Financial Care Counselor. I am so excited to be a part of the Duke PHMO team because they realize how important our patients are, and we strive to provide exceptional service to every patient that we come in contact with.

I graduated from Johnson C. Smith University in Charlotte, NC (J-C-S-U!!!) with a bachelors degree in Political Science, and I'm currently working on a masters degree in Public Administration from Strayer University. I have been a part of Duke Health since 2015, and my absolute favorite part of working for Duke is caring and assisting my patients with their healthcare needs. Originally from Puerto Rico, I started my healthcare journey while serving in the USAF as a health care technician. After my service, I started interpreting with a NC-based company in diverse medical settings. After interpreting, I stared working at Community Health Centers, finding interest in helping under-insured and uninsured populations.
I have spent the last two years working in mental health, and I am now in the process of earning my Masters of Public Health. Someday, I hope to make a difference in the health equity field. I have always known I wanted to help others, finding anything that could help them maintain a healthy and safe lifestyle, so I enjoy that I get to do what I love.

I was born and raised in Alabama. I am a graduate of North Carolina Central University with a B.S. in Family and Consumer Sciences. I have also earned two Associates Degrees with certifications as a Medical Assistant and Occupational Therapy Assistant (OTA). I have worked in healthcare and in public and private education. I enjoy watching action movies and shows like CSI and NCIS, taking long walks, spending time with my husband and two beautiful daughters, and enjoying holidays with extended family. My favorite vacation destination is the beach, especially the Caribbean.
I am proud to be a part of DukeWELL. I have been in health care for over 20 years, and I love it.
Since 1996, I have collaborated with learners in traditional and non-traditional classrooms, including community college, university, elementary, intergenerational, homeschool, and early childhood. The contiguous thread throughout my work is to co-create mindsets, heart-sets, environments, and practices that enliven learning and wellness among young children and adults. In my new role as a Pediatric Community Health Worker with Duke, I hope to expand the ways I am able to contribute to integral health.
I am very excited to be working with DukeWELL. I have experience with CNA work, Phlebotomy, and Health Unit Coordinating. I have worked in the medical field for many years, working with Geriatrics and Pediatrics. I look forward to helping all of our DukeWELL clients.
"I was born in California and raised in Shelby, North Carolina. I graduated from UNC-Chapel Hill in 2009 (Go Tarheels!) and earned a Master's in Public Administration from North Carolina Central University in 2017 (Go Eagles!). I enjoy trying new foods, great music, reading, traveling, softball, and trying to salsa dance. I have a daughter who keeps me on my toes. Spending time with her and my other family members and friends truly makes me happy. "
Dietitians are licensed staff who make sure your meals and nutrition are right for managing your chronic illness.
I have been a registered dietitian for over 30 years. After graduate school, I did an internship at Duke and then worked as the Relief Dietitian for a year before taking a job with a group of endocrinologists in a weight loss program for three years. I returned to Duke and worked in the General Clinical Research Center (Rankin Ward, Duke South, Blue Zone) and then transitioned to the Department of Pediatrics, Division of Medical Genetics, when the Genetic/Metabolic Clinic began. I was at Duke for 22 years before taking a job with a pharmaceutical company, where I have worked for the past 7 years. I am compassionate about all aspects of patient care, and I am so happy to be back at Duke once again!
I am a North Carolina native. I earned my Bachelor of Science in Public Health Nutrition from UNC Chapel Hill and my Master of Science in Nutrition from UNC Greensboro. I previously developed nutrition education programs for community settings. With my husband, I enjoy traveling, outdoor activities, cooking, spending time with friends and family, and being active in our church.
"I am originally from Richmond, VA. I completed my undergraduate degree in dietetics from Virginia Tech (Go Hokies!) and a master’s degree in public health at George Mason. I previously worked in inpatient care. My husband and I relocated to the Triangle in 2017, and I am now a dietitian care manager serving Medicaid and Medicare patients. In my spare time, I enjoy traveling, yoga and dance, crocheting, and spending time with friends and family."
"I am from the Washington, D.C. suburbs, but have lived in North Carolina since 2013 and now consider it home. I grew up in a big Greek family where we loved eating and talking about food. I earned my Master’s in Public Health focusing on nutrition from UNC. Today, I am passionate about helping people get access to healthy, tasty and culturally-appropriate foods... and empowering them to improve their health through good nutrition! I also love cooking, going on outdoor adventures and binge-listening to true crime podcasts."
I graduated from the UNC-Chapel Hill Eshelman School of Pharmacy in 2007 and am a Board Certified Pharmacotherapy Specialist (BCPS). Prior to joining Duke, I worked at Dorothea Dix and then practiced as a clinical pharmacist practitioner (CPP) managing lipid and anticoagulation clinics in various Research Triangle locations. I also serve on the advisory board of Pharmacist Moms, a national group dedicated to advocacy for women in pharmacy. My specific areas of interest are cardiovascular risk reduction and mental health.
"I am a clinical pharmacist and adjunct assistant professor of clinical education at UNC Eshelman School of Pharmacy. I graduated from the UNC Eshelman School of Pharmacy in 2004 and worked as a community pharmacist from 2004-2010. I completed my residency at WakeMed Health & Hospitals. My interests include working with patients to help control their diabetes, high blood presure and high cholesterol."
Dr. Jackson Carter serves Duke employees and their dependents as senior clinical account executive with Express Scripts. She manages the Duke pharmacy benefit and works with physician leadership on realizing cost savings, improving patient care, and increasing patient safety. Through her efforts at Duke, Dr. Jackson Carter was awarded a "Pharmacy Benefit Management Institute Excellence Award" in 2016 for innovative care management strategies. Dr. Jackson Carter graduated from UNC Chapel Hill with a degree in biology and doctorate in pharmacy. She completed a residency in ambulatory care with Mission Hospitals and holds her ACCP board certification in ambulatory care.
I obtained my pharmacy degree from the UNC Eshelman School of Pharmacy, as well as a PGY-1 with a focus in Ambulatory Care, and I am a Board Certified Pharmacist in Ambulatory Care. Prior to coming to Duke, I worked as a Clinical Pharmacist at Advocate Medical Group in Chicago, Illinois, managing patients with diabetes, hypertension, heart failure, hyperlipidemia, and on anticoagulation. I also worked as an Assistant Professor at Chicago State University. I am interested in chronic disease states and drug development research. Being a chronic disease state pharmacist has allowed me to help and educate patients regarding preventative medicine. It also has given me the opportunity to educate and mold young pharmacists regarding our profession's role.
"I am board certified in both ambulatory care and geriatric pharmacy practice and serve as assistant professor at the UNC Eshelman School of Pharmacy. I graduated from the Eshelman School of Pharmacy at UNC-Chapel Hill and completed a pharmacy residency in ambulatory care at Mission Hospital and the Mountain Area Health Education Center (MAHEC) in Asheville. Prior to joining Duke Health in 2012, I practiced in an outpatient clinic at Mission Hospital."
Pharmacy technicians are licensed staff who can help you on the phone or in your home. They make sure your medicine list is up-to-date, that you are taking medicines safely, and that you are able to get refills on time.
"I am a certified pharmacy technician who graduated from UNC-Chapel Hill in 2013 with a Bachelors of Arts in Biology. I have retail pharmacy experience dates back to 2009 at Kerr Drug and continued working in the pharmacy through college before becaming nationally certified in February 2012. I provide general pharmacy and administrative support to DukeWELL care managers and conduct home visits to Medicare patients in Wake and Durham Counties in order to obtain accurate medication histories and support safer medication use."
"I am a certified pharmacy technician with over 12 years of experience in sales and pharmacy. I have worked in a variety of pharmacy settings, ranging from IV chemotherapy preparation and medication procurement to prior authorizations and patient assistance. I attended Appalachian State University, where I graduated with a bachelor's degree in cellular and molecular biology with minor concentrations in chemistry and business. I currently serve DukeWELL patients in Orange, Durham and Alamance Counties."
Virginia is a Certified Pharmacy Technician II, who is currently enrolled at the University of North Carolina Greensboro pursuing a degree in Public Health Education. Her pharmacy experience began in February 1998 at Wal-Mart Pharmacy. In January 1999, she began her career at Duke where she was enrolled in the Duke Pharmacy Tech Program. Upon completion, she began working with the Duke Pharmacy Department preparing IV chemotherapy drugs for patients receiving outpatient oncology treatment. In March 2000, she became nationally certified with the Pharmacy Technician Certification Board. During her 20 years of employment with Duke, she has also worked at CVS, Rite-Aid, and Wal-Mart pharmacies. She currently conducts home visits to DukeWELL patients living in surrounding counties.
"I am a certified pharmacy technician who attended Virginia Union University and Chowan University, where I studied allied health sciences with a minor in chemistry. My retail pharmacy experience began in 2003, when I worked at a local pharmacy and soda shop in my community. Since then, I have worked in a variety of pharmacy settings, including supporting pediatrics, Duke’s Department of Pharmacy Continuity of Care Program, pharmacy billing, and even working in our U.S. Army Military Treatment Facility pharmacies. I currently provide home visit support to DukeWELL patients who live in Wake, Durham, Person, Vance and Granville Counties."
Occupational Therapists assist people from across a variety of lifespans to engage in the things they want and need to do through the therapeutic use of daily activities (occupations). Occupational therapy practitioners enable people of all ages to live life to its fullest by helping them promote health and prevent—or live better with—injury, illness, or disability.
I am a 2012 graduate from NC Wesleyan College with a B.S. in Exercise Science and 2015 graduate from Howard University (the real HU!) where I received my Master’s degree in Occupational Therapy. I am also a Certified Lymphedema Therapist and initially began my OT career in the adult inpatient acute care hospital setting here at Duke Health. I love being an OT, because I have the opportunity to be creative when incorporating the interests of my patient’s and developing goals that improve their independence, as well as quality of life. I love making a difference, advocating for others, and seeing our community thrive with impactful encounters. I am elated to be a part of the DukeWell team!
